NSAIDs commonly cause an increase in systolic blood pressure by 3 to 6 mmHg in older adults. While this may seem modest, it is enough to push many seniors from controlled to uncontrolled hypertension. Elevated blood pressure increases the risk of stroke, heart attack, and kidney disease progression.
The mechanism involves NSAID-induced reduction of vasodilatory prostaglandins and sodium retention, leading to vasoconstriction and volume expansion. This effect can blunt the efficacy of blood pressure medications such as ACE inhibitors, ARBs, and diuretics, complicating hypertension management.
For seniors relying on antihypertensive therapy, even a small blood pressure rise can have significant clinical consequences. Regular blood pressure monitoring is essential if NSAIDs are used, and adjustments to medications may be required.
4. Atrial fibrillation risk
Atrial fibrillation (AFib), a common and potentially dangerous irregular heart rhythm, has been linked to NSAID use. NSAIDs increase systemic inflammation and fluid retention, which can alter electrical conduction in the atria and precipitate AFib episodes.
In seniors, AFib raises the risk of stroke and heart failure, and often requires long-term anticoagulation therapy. Since NSAIDs can also affect platelet function and clotting pathways, their use in AFib patients demands careful consideration and coordination with healthcare providers.
5. Kidney damage risk
NSAIDs carry a well-established risk of kidney injury, particularly in older adults or those with pre-existing chronic kidney disease (CKD). By inhibiting prostaglandins that maintain adequate kidney blood flow, NSAIDs can cause acute kidney injury or accelerate CKD progression.
Signs of NSAID-induced kidney damage include reduced urine output, swelling, fatigue, and elevated blood creatinine levels. This risk is higher with prolonged use and in combination with other medications that affect kidney function, such as diuretics or ACE inhibitors.
For seniors, regular monitoring of kidney function tests is crucial if NSAIDs must be used, and alternative pain management strategies should be prioritized whenever possible.
